Red Yeast Rice

Rice fermented with red yeast contains monacolin K — chemically identical to lovastatin. Effective for cholesterol; carries the same risks as a statin.

Red yeast rice is a TCM food made by fermenting rice with Monascus purpureus yeast. The fermentation produces monacolin K — the same molecule as the prescription statin lovastatin. So this isn't really a 'natural alternative' to statins; it IS a statin, in unregulated form. The dose of active drug varies widely between products, so dosing is unpredictable. Also potentially contaminated with citrinin (nephrotoxic). Genuinely effective for lipid lowering, but the muscle-pain and liver-enzyme risks are the same as any statin. Take only with provider monitoring.

Side effects
Same as statins: muscle pain, weakness, fatigue, liver enzyme elevation. Rhabdomyolysis (rare). Cognitive complaints in some.
Interactions
Same as prescription statins. CYP3A4 inhibitors (grapefruit, certain antibiotics, antifungals, calcium channel blockers) raise blood levels dangerously. Anticoagulants — interaction. Niacin — additive muscle risk.
Contraindications
Already on a statin. Active liver disease. Pregnancy / breastfeeding. Recent rhabdomyolysis. Heavy alcohol use.
Lab markers
Lipid panel, liver enzymes (ALT, AST), CK (creatine kinase) if muscle pain develops. Same monitoring as prescription statins.
Food sources
Used as a food colorant and flavoring in Chinese cuisine — Peking duck, char siu pork — but in much smaller medicinal doses.

Pairs with

CoQ10 (mandatory — statin-like drugs deplete CoQ10). Bergamot, omega-3 (lipid stack).
