Medium-chain triglycerides are fats with 6–12 carbon chains (versus the 14–24 of most dietary fats). They skip lymphatic absorption, go directly to the liver via the portal vein, and are rapidly converted to ketones — alternative fuel for the brain and muscles, especially useful when glucose is restricted. C8 (caprylic acid) and C10 (capric acid) are the most ketogenic. The famous 'butter coffee' effect comes from MCTs. Most useful for ketogenic dieters, cognitive support during intermittent fasting, and as quick energy that doesn't require digestion. Start small — full doses on a naive gut produce a fast and unpleasant experience.

Form
C8-only (caprylic acid) MCT oil — most ketogenic, cleanest. C8/C10 blend — slightly cheaper, still effective. Coconut oil contains ~50% MCTs but mostly C12 (lauric acid), which acts more like a long-chain fat — less effective per gram. MCT powder for travel / mixing into drinks.
Dose
1–2 teaspoons (5–10 g) initially, building to 1–2 tablespoons (15–30 g) daily as tolerated.
Timing
AM in coffee for sustained cognitive energy. Pre-workout. With meals to slow digestion and improve satiety.
Schedule
Daily as part of a ketogenic / low-carb diet. Anytime for cognitive use.
Side effects
'Disaster pants' — the classic over-dose effect. Cramps, urgent diarrhea, GI upset. Start small (1 tsp) and titrate up. Most people adapt over 1–2 weeks.
Interactions
Generally clean. Coumadin / warfarin — theoretical mild interaction. Some diabetes medications — additive on glucose control.
Contraindications
Liver disease (rare — MCTs are processed by the liver). Active gastroparesis. Caution in those with extreme caloric restriction or eating disorders (ketones can suppress hunger).
Lab markers
Blood ketones (beta-hydroxybutyrate) for ketogenic tracking. Lipid panel periodically.
Food sources
Coconut oil and palm kernel oil contain MCTs naturally (mostly C12, less effective). Whole coconut and full-fat dairy contain small amounts. MCT oil itself is a refined product, not a whole food.
