A probiotic yeast (not bacteria) — which is exactly why it works alongside antibiotics. The most evidence-backed prevention for C. difficile and antibiotic-associated diarrhea.

Strong for: antibiotic-associated diarrhea prevention (~50% reduction), C. difficile recurrence prevention (the only probiotic with consistent C. diff data), traveler's diarrhea, acute pediatric gastroenteritis. Effects: within days.

S. boulardii is a yeast, not a bacterium — which means it's unaffected by antibacterial drugs. Take it simultaneously with antibiotics (unlike bacterial probiotics, which need to be timed apart). It produces antimicrobial substances active against pathogenic bacteria, modifies host immune signalling in the gut, and disrupts C. difficile toxin attachment. Multiple meta-analyses confirm meaningful protection against C. diff and antibiotic-associated diarrhea. Discovered by French microbiologist Henri Boulard, who noticed locals in Southeast Asia chewing lychee skins to treat diarrhea — the active organism turned out to be a yeast on the fruit.

Form
Saccharomyces boulardii (CNCM I-745) — the well-studied strain. Florastor is the leading brand. Generic versions also fine if strain is specified.
Dose
250–500 mg (5–10 billion cells) 2x daily during antibiotic use. 250 mg/day for general gut support.
Timing
Can be taken with antibiotics (no need to space). With food.
Schedule
Throughout antibiotic course + 7 days after. Travel: starting 3 days before, continuing through trip.
Side effects
Mild gas, constipation occasionally. Rare yeast fungemia (almost exclusively in critically ill patients with central venous catheters).
Interactions
Antifungal medications (fluconazole, etc.) — antifungal kills probiotic yeast. Generally clean otherwise.
Contraindications
Central venous catheter in place — risk of rare but serious fungemia. Critical illness / ICU patients. Severe immune compromise (HIV, transplant, neutropenia). On systemic antifungal therapy.
