What it isA measurement of the average diameter of LDL particles. The particles come in a spectrum from large and buoyant to small and dense.

DoesSmall dense LDL penetrates the artery wall more readily and oxidizes more quickly than large buoyant LDL, so size shifts the per-particle atherogenicity.

GoodLarge buoyant LDL (Pattern A) is the less harmful phenotype — fewer particles slip into the subendothelial space.

BadSmall dense LDL (Pattern B) is the metabolic-syndrome phenotype — high triglycerides, low HDL, insulin resistance — and carries elevated cardiovascular risk per particle.
