LDL Particle Number (LDL-P)

Advanced wellness

also ldl-p, nmr particle count

What it isA direct count of LDL particles measured by nuclear magnetic resonance spectroscopy. Conceptually parallel to ApoB — both count atherogenic particles, just by different methods.

DoesQuantifies the supply of LDL particles available to deposit cholesterol into vessel walls.

GoodLow LDL-P means few particles knocking on the endothelium and predicts low cardiovascular risk regardless of how much cholesterol each particle carries.

BadElevated LDL-P drives atherosclerosis. When LDL-C and LDL-P disagree, the higher of the two carries the risk — common in insulin resistance.

Standard
<1000 nmol/L optimal; >2000 nmol/L high.
Optimal
500–900 nmol/L (lower is good)
Modifiers
Discordance with LDL-C rises with insulin resistance, small dense LDL.

How to optimize

Lifestyle
Zone 2 cardio, resistance training, weight loss.
Diet
Cut saturated fat, refined carbs; add soluble fiber, olive oil, nuts.
Supplements
Plant sterols (2g/day), psyllium (10g/day), berberine (500mg 3×/day), bergamot extract (500mg 2×/day).
Peptides
Drugs
Statins, ezetimibe, PCSK9 inhibitors, bempedoic acid.
