What it isApoB is the structural protein on the surface of every atherogenic lipoprotein particle — one ApoB molecule per particle. Measuring ApoB therefore gives a direct count of atherogenic particles.

DoesEach ApoB-containing particle is a candidate to deposit cholesterol into the artery wall. The count of these particles, not the cholesterol they carry, drives atherosclerosis.

GoodLow ApoB means few atherogenic particles in circulation and is the single strongest lipid predictor of low cardiovascular risk.

BadElevated ApoB causally drives atherosclerosis, heart attack, and stroke — even when LDL-C looks reassuring (a common discordance in metabolic syndrome).
