The cofactor for neurotransmitter synthesis — serotonin, dopamine, GABA all need B6 to be made. Often the missing piece in mood, sleep, and PMS.

Strong for PMS symptoms (especially mood), nausea in pregnancy, homocysteine reduction, and seizure disorders in deficient individuals. Adjunct in depression (especially when paired with magnesium and B12/folate). Effects: 2–6 weeks for mood / PMS; faster for nausea.

B6 is required for over 140 enzymatic reactions, but its most relevant role is in converting amino acid precursors into neurotransmitters: tryptophan → serotonin, tyrosine → dopamine, glutamate → GABA. It's also a key methylation cofactor (alongside B12 and folate) for clearing homocysteine. Deficiency is more common than people realise — particularly in women on hormonal contraceptives (which deplete B6), those on long-term diuretics, alcohol users, and anyone with chronic inflammation. The classic supplement form (pyridoxine HCl) is fine in modest doses but at higher doses paradoxically causes neuropathy. The active form, P5P (pyridoxal-5-phosphate), works at lower doses and avoids the toxicity risk.

Form
P5P (Pyridoxal-5-Phosphate) — the active coenzyme form. Best for anyone with MTHFR variants or who has trouble with regular pyridoxine. Pyridoxine HCl — the standard cheap form, fine at modest doses.
Dose
P5P: 25–50 mg/day. Pyridoxine HCl: 25–100 mg/day. Don't exceed 100 mg/day chronically — risk of paradoxical sensory neuropathy at sustained high doses.
Timing
AM with food. Mildly energising for some.
Schedule
Daily.
Side effects
Peripheral neuropathy — tingling, numbness, balance issues — at chronic doses above 100 mg/day, sometimes lower in sensitive individuals. The neuropathy can be slow to reverse. Stay under 100 mg and switch to P5P if running long-term.
Interactions
Levodopa (Parkinson's drug) — B6 dramatically increases peripheral breakdown of levodopa, reducing efficacy. Hormonal contraceptives deplete B6. Some antibiotics (isoniazid, cycloserine) deplete B6. Anti-seizure drugs interact bidirectionally.
Contraindications
Levodopa therapy (without carbidopa). Pre-existing peripheral neuropathy (caution — could worsen). History of B6-induced neuropathy.
Lab markers
Plasma P5P (active form) — better than total B6. Homocysteine elevates when B6, B12, or folate is low. Symptomatic markers: PMS severity, mood, neuropathy signs.
Food sources
Chickpeas, beef liver, tuna, salmon, chicken breast, potatoes (with skin), bananas, pistachios, sunflower seeds, avocado. Cooking destroys some B6 — raw or lightly cooked sources retain more.
