Topical Retinoids / Retinol
The single most evidence-backed topical for skin aging and acne. Decades of RCT data — nothing else comes close.
The most evidence-backed topical in dermatology. Strong for fine lines, photoaging, pigmentation, acne, and stretch marks. Effects: 12+ weeks for visible changes; 6+ months for full benefit.
Retinoids are vitamin A derivatives that bind nuclear receptors in skin cells to drive cell turnover, collagen synthesis, and gene expression changes. The category spans potency: retinyl esters (mildest, OTC) → retinol → retinaldehyde → tretinoin (Rx). Prescription tretinoin (Retin-A) is the gold standard, but well-formulated OTC retinol gets you most of the way with less irritation. Strongest evidence in dermatology for any topical: fine lines, photoaging, pigmentation, acne, and texture all improve over 12+ weeks. Build up slowly.
- Form
- Tretinoin 0.025–0.1% (Rx) — gold standard. Adapalene 0.1% (Differin, OTC) — gentler, good for acne. Retinol 0.3–1% (OTC) — well-formulated brands (CeraVe, Skinceuticals, Paula's Choice). Retinaldehyde — middle ground.
- Dose
- Pea-sized amount to entire face, 2–3x per week initially, building to nightly. Start very low concentration.
- Timing
- PM only — retinoids degrade in sunlight and increase sun sensitivity.
- Schedule
- Start 2x/week, build to nightly over 4–8 weeks. Daily SPF in AM is non-negotiable.
- Side effects
- The retinoid uglies — redness, peeling, dryness, breakouts, sensitivity in weeks 2–6. Resolves with persistence. Sun sensitivity. Skin barrier compromise if overused.
- Interactions
- Other actives — don't combine with AHAs/BHAs in same routine (irritation). Benzoyl peroxide deactivates tretinoin (separate by 12 hours). Vitamin A supplements + tretinoin = additive toxicity.
- Contraindications
- Pregnancy and breastfeeding (teratogenic — true of oral isotretinoin; topical risk is lower but the standard advice is to avoid). Active eczema / rosacea (use gentler forms). Recent waxing / dermabrasion.
