Nattokinase
A fibrin-dissolving enzyme from fermented soybeans (natto). Modestly lowers blood pressure, dissolves abnormal clots, and — at high doses — has shown carotid-plaque and cholesterol reduction in trials. The catch: the dose does the work, and typical capsules are far too small.
Moderate evidence for blood pressure reduction (~5–10 mmHg in hypertensives), modest improvement in fibrinogen and von Willebrand factor, and mild antiplatelet effect. Strong epidemiological signal from natto-consuming populations. Effects: 4–8 weeks for BP and fibrinogen. For carotid plaque and cholesterol, a large 2022 trial (1,062 people, 10,800 FU/day, 12 months) reported ~36.6% plaque reduction and improved lipids — dose-dependent (3,600 FU did nothing) and intriguing, but not independently replicated and lacking robust placebo control, so treat it as preliminary.
Nattokinase is a serine protease isolated from natto — the traditional Japanese fermented soybean dish. It selectively breaks down fibrin (the protein scaffold of blood clots) and the clot-stabilising plasminogen activator inhibitor. Clinical effects: modest reduction in blood pressure, lowering of fibrinogen levels, and possible dissolution of abnormal microclots. It's been part of the Japanese diet for over 1,000 years (natto consumption tracks with that country's exceptional cardiovascular health) and is now used in functional cardiovascular protocols globally — including in long-COVID microclot protocols. On cholesterol and arterial plaque specifically, the effect is strongly dose-dependent: the largest trial (1,062 people, 12 months; Frontiers in Cardiovascular Medicine, 2022) found ~36.6% average carotid-plaque reduction plus better lipids at 10,800 FU/day, while 3,600 FU/day did nothing — and an earlier head-to-head (Ren 2017) reported it shrank plaque more than simvastatin while raising HDL. Read with honest eyes, though: these standout results come largely from one research orbit, aren't independently replicated, and the strongest designs lacked a true placebo (a separate placebo-controlled trial, NAPS, found no effect on plaque).

The most-cited human evidence and by far the largest — but it comes largely from one research orbit, isn't independently replicated, and had no true placebo arm. Read as promising and dose-dependent, not proven.
- NAPS trial — randomised, placebo-controlled (PubMed) — The honest counterweight: a placebo-controlled trial found no effect of nattokinase on plaque progression. The gold-standard design (blinded, placebo) is exactly what the eye-catching open-label studies lack — which is why the overall grade stays cautious.
- Form
- Standardised nattokinase measured in FU (fibrin units). Common capsules are 2,000–4,000 FU — fine for general and blood-pressure use, but well below the 6,000–10,800 FU/day the cholesterol-and-plaque trials used. NSK-SD is the patented form used in most studies.
- Dose
- 2,000–4,000 FU/day for general cardiovascular / blood-pressure use. For the cholesterol and plaque effects, trials used far higher doses (~6,000–10,800 FU/day) — lower doses (≤3,600 FU) showed no effect. High therapeutic doses meaningfully thin the blood, so use them only under medical supervision.

- Empty stomach (30–60 min before food, or 2 hours after). AM or PM both work.

- Side effects
- Generally well-tolerated. Mild GI upset, headache. Increased menstrual flow in some.
- Interactions
- Anticoagulants and antiplatelets (warfarin, DOACs, aspirin, clopidogrel) — additive blood-thinning, potentially significant. Discuss with prescriber. Antihypertensives — additive BP lowering.
- Contraindications
- Bleeding disorders. Pre-surgery (stop 7–14 days before). Soy allergy (some products derived directly from natto). Pregnancy / lactation. Recent stroke or active bleeding.

- Fibrinogen, D-dimer (if monitoring microclot protocols). PT/INR if on warfarin. Blood pressure.
- Food sources
- Natto — fermented soybeans (the only practical food source). Stinky, sticky, and acquired-taste, but eating natto regularly delivers nattokinase plus the highest dietary K2 (MK-7) on earth.
