Strong evidence for full-dose lithium in bipolar disorder (gold standard). Emerging evidence at low / orotate doses: a small trial showed slowed cognitive decline in Alzheimer's, ecological studies link drinking-water lithium to reduced suicide and dementia rates, and anecdotal reports support mild mood / sleep benefit. Effects: 4–8 weeks for subjective; longer for cognitive / longevity claims.

Lithium is the lightest metal and an element with quietly fascinating biology. Pharmaceutical lithium (lithium carbonate, 600–1,800 mg/day) is the gold-standard mood stabiliser in bipolar disorder. But at much lower doses — 1–20 mg of elemental lithium daily — research suggests benefits for mood, sleep, neuroprotection, and possibly longevity. Population studies show areas with higher lithium in drinking water have lower rates of suicide, violence, and dementia. The supplemental form (lithium orotate) delivers a small but measurable amount of bioavailable lithium without the side-effect profile of the prescription doses. Sits in an interesting middle ground between supplement and micro-dose medication.

Form
Lithium orotate — bound to orotic acid, which appears to improve cellular uptake at low doses. Standard products deliver 5 mg elemental lithium per capsule. Lithium aspartate is an alternative. Avoid lithium carbonate / citrate (the Rx forms) without medical supervision.
Dose
1–10 mg elemental lithium per day. Start at 1–5 mg. Doses above 20 mg/day move toward needing the lab monitoring that prescription lithium requires.
Timing
PM with food (some find it calming and pro-sleep). AM also fine.
Schedule
Daily.
Side effects
Generally well-tolerated at low doses. Mild GI upset, hand tremor, thirst, mild cognitive blunting in sensitive people. Far less than pharmaceutical lithium.
Interactions
NSAIDs raise serum lithium levels (relevant at high doses, less so at supplement levels). ACE inhibitors and thiazide diuretics — same. Caffeine increases lithium clearance. Inositol opposes lithium pharmacologically.
Contraindications
Kidney impairment (lithium is renally cleared). Pregnancy (Ebstein anomaly risk — prescription doses; supplement-level risk lower but unclear). Severe thyroid disease (lithium suppresses thyroid). Active dehydration (concentrates lithium).
Lab markers
Standard lithium-orotate doses don't require monitoring. If running higher (15+ mg/day) or for extended periods: serum lithium (target <0.3 mEq/L for supplemental use; 0.6–1.0 mEq/L is the pharmacological range). TSH and creatinine annually as a baseline of caution.
Food sources
Trace amounts in drinking water (variable by region) and some grains, vegetables, and seafood. Not enough to deliver supplemental doses through food.

Pairs with

Omega-3 DHA — synergistic neuroprotection. Vitamin B12, folate (lithium increases need for both). Inositol — opposes lithium's action (don't combine if relying on lithium's mood effect).
