Boswellia serrata is the Indian variety of the same frankincense tree used in incense for 3,000 years. Its bioactive boswellic acids — especially AKBA (acetyl-11-keto-β-boswellic acid) — selectively inhibit 5-lipoxygenase, an inflammatory enzyme that NSAIDs and steroids don't touch. This gives boswellia a different mechanism than most anti-inflammatories, complementary to ibuprofen / curcumin / fish oil. Best evidence: knee osteoarthritis (comparable to NSAIDs in head-to-head trials), inflammatory bowel disease (ulcerative colitis, Crohn's), and asthma. Often combined with curcumin in joint formulas.

AprèsFlex / AflapIN (standardised to 20% AKBA) — most-studied bioavailable form. 5-Loxin (30% AKBA) — also clinically supported. Plain boswellia 65% — older, less concentrated.
Dose
100–250 mg/day of AprèsFlex / 5-Loxin. 300–500 mg 2–3x daily of standard 65% extract.
Timing
With food. AM and PM split for higher doses.
Schedule
Daily, ongoing.
Side effects
Generally well-tolerated. GI upset, nausea, mild diarrhea. Rare skin rash.
Interactions
NSAIDs — additive (helpful for pain, but combined GI risk). Immunosuppressants — mild interaction (boswellia is mildly immune-modulating).
Contraindications
Pregnancy (may stimulate menstruation). Active IBD flare without provider input.

Not standard. CRP, ESR for inflammation tracking.
Food sources
Frankincense resin is traditionally chewed in some cultures but isn't part of typical diet.
