Everything in this guide reduces to a small, shared foundation plus a decision about whether to add medical treatment. Losing weight is the short project; holding it is the long one, and the two need different skills.

The base under any approach

Whether or not you ever use a drug or surgery, the same base does the work: a modest, sustainable deficit; protein-forward, fiber-rich, mostly whole foods; resistance training plus daily steps; protected sleep; and self-monitoring to catch drift early. Medications and operations amplify this foundation; they don’t replace it — the long-term outcome still tracks the habits.

The maintenance playbook

The honest odds

On average, more than half of lost weight returns within ~2 years and ~80% within 5 — but a meaningful ~20% keep ≥10% off long-term, and even partial maintenance delivers real health benefit. The maintainers share a profile: ~an hour a day of activity, frequent self-weighing, a consistent eating pattern across weekdays and weekends, and catching small regains early. And it gets easier — once weight is held ~2 years, the odds of regain fall sharply. Evidence ●●●●○

Reset the goal

Aim for a 5–10% loss you can hold, tracked in waist and blood markers as much as pounds, rather than a maximal loss you can’t. The extra weight squeezed out by an aggressive diet is disproportionately the weight that comes back.

Where medical treatment fits

Lifestyle first, always. If the foundation is genuinely in place and falling short, and BMI is ≥30 (or ≥27 with a weight-related condition), a GLP-1 or other medication becomes a reasonable clinical conversation. For severe obesity (roughly BMI ≥35, or ≥30 with metabolic disease), surgery offers the most durable result. Increasingly these are sequenced and combined — but every medical step is a decision to make with a clinician, not from a guide.

The real skill

Dozens of approaches can get weight off — that’s the easy part. The skill that decides the outcome is the durable, mildly effortful system — activity, protein, monitoring, consistency — that holds it there against a body built to get it back.
