Why it torments some and not others

The aim of good treatment is not silence but habituation: the brain filtering the sound back into the background, the way you stop hearing a fridge hum or a ticking clock. Most people habituate naturally over months; therapy speeds and deepens it. Where the leverage is

Because the distress lives in the attention-and-emotion loop, the best-evidenced treatments target that loop — therapy, sound enrichment, hearing aids — rather than chasing a way to erase the sound. This is not 'it's all in your head' in the dismissive sense; it's a real, physical brain process. That it's a brain process is exactly why it responds to retraining.

Habituation, not silence

'Habituation, not silence' is the frame that reduces suffering fastest. Fighting to eliminate the sound tends to feed the very attention loop that keeps it loud — whereas letting it become background, with help, is both achievable and what actually happens for most people.
