The functional & biohacker lens

A large 'optimisation' world sits alongside mainstream care — some of it genuinely useful, some oversold, some risky. Worth engaging with honestly rather than dismissing, because it's where a lot of men actually get their information. Attributed and graded.

What's reasonable

The defensible core

The central biohacker idea — optimise by removing suppressors and aim for a healthy upper-normal level — is sound and matches this guide. Prioritising sleep and fat loss, cutting heavy alcohol, correcting vitamin D or zinc deficiency, and using ashwagandha for stress are all defensible. Much of the good functional-medicine advice here is simply the well-evidenced basics taken seriously.

Enclomiphene as a 'TRT alternative'

Genuinely evidence-based — with caveats

Increasingly promoted by functional and telehealth clinics, enclomiphene (a cleaner isomer of clomiphene) raises your own testosterone while preserving sperm — for secondary hypogonadism in a man who wants fertility, that's a legitimate, evidence-based option, not hype. The honest caveats: it's still a prescription needing monitoring (not a supplement), it works only when the pituitary/testes can respond, and its long-term data are thinner than TRT's. What's oversold or risky

The stacks and the AI reflex

The viral fadogia + tongkat 'T-booster' stack (popularised on major podcasts) rests on thin and animal-only data, with a real toxicity flag on fadogia. Routine aromatase inhibitors to 'dial in estrogen' do more harm than good (see the monitoring section). DHEA and pregnenolone do little for testosterone in men. And chasing supraphysiologic 'high-normal' numbers with ever-climbing doses quietly drifts toward the anabolic-steroid risk profile. Credit the good basics; be sceptical of the stacks and the escalation. Evidence ●○○○○ for the boosters

Keep the signal, drop the noise

The defensible half of the optimisation world is the same unglamorous advice: sleep, lose fat, cut alcohol, fix deficiencies, and — where warranted and prescribed — use enclomiphene or TRT properly. The noise is the branded stacks, the estrogen-crushing reflex, and the push toward supraphysiologic numbers.
