The exciting direction

The most exciting idea is antigen-specific tolerance — 'inverse vaccines' that teach the immune system to stop attacking the thyroid specifically, rather than blanket immunosuppression. The proof of concept already exists in the sister disease Graves' (a peptide therapy improved ~70% of patients in an early trial); Hashimoto's-specific versions are still in the lab. The most futuristic: lab-grown thyroid tissue. Human stem-cell-derived thyroid 'organoids' can make hormone and, transplanted into mice with no thyroid, restored their levels.
