Gut, stress, sleep & the myths

Around Hashimoto's swirls a huge functional-medicine industry of gut protocols, 'adrenal fatigue,' and virus-hunting. Some of it points at real biology; much of it is unproven. Here's what's genuinely worth doing — and what to be skeptical of.

The gut: treat the real thing

Celiac, H. pylori, SIBO

The gut-thyroid link is real, but the concrete, evidence-based version isn't a 'leaky gut protocol' — it's finding and treating an actual condition. Screen once for celiac; test and treat H. pylori or SIBO if you have symptoms. Probiotics and fibre help gut comfort and quality of life but don't move your thyroid numbers — worth it for symptoms, not as thyroid treatment. Evidence ●●●○○ (celiac screening ●●●●●)

Stress & sleep are real — 'adrenal fatigue' isn't

The genuine levers

Chronic stress genuinely modulates autoimmunity, and stress reduction (mindfulness, yoga, breathwork) reliably improves symptoms and quality of life — do it. But 'adrenal fatigue' is not a recognised diagnosis, and its saliva-cortisol panels and adrenal supplements aren't validated (they can even mask the real cause — under-treated thyroid, sleep apnoea, depression). And fix sleep: hypothyroidism and poor sleep feed each other, so if you snore or stay exhausted despite a normal TSH, get screened for sleep apnoea. Evidence ●●●○○

Smoking, weight & honest expectations

Two realities

A curiosity: smoking oddly associates with lower Hashimoto's risk — but it harms you overall and worsens the related Graves' disease, so still quit (just get your thyroid checked afterward, as risk briefly rises). And a hard truth about weight: hypothyroidism causes only modest gain (much of it water), and treatment doesn't melt it off — never push your dose into the hyperthyroid range to lose weight (it risks your heart and bones and doesn't work). Be skeptical of the elaborate protocols

The proven lifestyle wins are specific and unglamorous: treat a real gut condition, reduce stress, sleep well (rule out apnoea), move moderately. 'Detoxes,' EBV 'protocols,' and adrenal-fatigue supplements are mechanistically interesting but not proven Hashimoto's treatments — spend your energy on the levers that are.
