The regression ladder
- Lower LDL, more regression
Serial ultrasound-inside-the-artery (IVUS) trials line up as a dose-response ladder measuring percent atheroma volume (PAV). REVERSAL: LDL 110 progressed (+2.7%), LDL 79 halted. ASTEROID: LDL 61 regressed plaque (−0.98%) — the first clear reversal. SATURN: both maximal statins regressed (~−1%). GLAGOV: adding a PCSK9 inhibitor drove LDL to 37 mg/dL and regressed plaque in 64% of patients, with benefit continuing down to LDL ~20 and no floor. Evidence ●●●●○
