Total Cholesterol

Routine annual

also cholesterol, total; tc

What it isTotal cholesterol is the sum of cholesterol carried in every type of lipoprotein particle — LDL, HDL, VLDL, and remnants. The liver makes most cholesterol; diet contributes a smaller fraction.

DoesCholesterol is a structural component of every cell membrane and the precursor to bile acids, steroid hormones, and vitamin D. It rides in lipoproteins because it's fat-insoluble.

GoodA moderate level reflects healthy production and clearance, with enough raw material for membranes and hormones.

BadAs a standalone number, total cholesterol is a crude predictor — what matters is which particles carry it. Two people with identical total cholesterol can have wildly different cardiovascular risk.

Standard
<200 mg/dL desirable; 200–239 borderline; ≥240 high.
Optimal
No widely-accepted optimal as a standalone — ApoB is the cleaner target.
Modifiers
Rises slightly with age and menopause; ketogenic diets often raise it; rises after illness or surgery.

How to optimize

Lifestyle
Zone 2 cardio, resistance training, weight loss in those with excess visceral fat.
Diet
Saturated fat raises it modestly; soluble fiber (oats, beans, psyllium) lowers it; cut ultra-processed food.
Supplements
Soluble fiber (psyllium 10g/day), red yeast rice, plant sterols (2g/day), omega-3 EPA+DHA (2–4g/day).
Peptides
Drugs
Statins, ezetimibe, bempedoic acid, PCSK9 inhibitors.
