Parathyroid Hormone (PTH)

Advanced wellness

also intact pth

What it isPTH is the master regulator of calcium balance, released by the four parathyroid glands when blood calcium falls.

DoesRaises blood calcium by pulling calcium from bone, activating vitamin D in the kidney to enhance gut absorption, and decreasing kidney calcium excretion.

GoodA mid-range PTH with optimal vitamin D and calcium signals balanced mineral homeostasis.

BadHigh PTH with high calcium signals primary hyperparathyroidism (often a benign adenoma) that drives bone loss, kidney stones, and cognitive symptoms. High PTH with low/normal calcium reflects vitamin D deficiency. Low PTH is rare.

Standard
15–65 pg/mL.
Optimal
20–40 pg/mL (narrow target)
Modifiers
Rises when vitamin D is low; elevated in chronic kidney disease.

How to optimize

Lifestyle
Sun exposure; weight-bearing exercise.
Diet
Adequate calcium from food; adequate vitamin D.
Supplements
Vitamin D3 to target 60–80 ng/mL; magnesium glycinate; vitamin K2 MK-7.
Peptides
Drugs
Parathyroidectomy for primary hyperparathyroidism; cinacalcet for secondary hyperparathyroidism in CKD.
