Hereditary Hemochromatosis (HFE)

What it isMutations in the HFE gene that disrupt hepcidin signaling and allow excess intestinal iron absorption. C282Y homozygosity is the classic high-penetrance form; compound heterozygotes (C282Y/H63D) and H63D homozygotes have lower penetrance.

DoesRemoves the normal brake on iron uptake from food, leading to progressive iron deposition in liver, pancreas, heart, joints, skin, and endocrine glands over decades.

GoodWild-type or low-penetrance genotype confers normal iron handling.

BadUntreated C282Y homozygotes can develop cirrhosis, hepatocellular carcinoma, cardiomyopathy, diabetes, arthritis, hypogonadism, and skin pigmentation. Detected early, complications are largely preventable.

Standard
Genotype reported across HFE variants.
Optimal
No widely-accepted optimal — the genotype is fixed; goal is normalizing iron stores (ferritin <50 ng/mL, transferrin saturation <45%) by phlebotomy in expressing carriers.
Modifiers
One-time test; menstruation and pregnancy delay iron loading in women; alcohol and vitamin C with meals accelerate it.

How to optimize

Lifestyle
Therapeutic phlebotomy is the mainstay; avoid alcohol; avoid raw shellfish (Vibrio vulnificus risk).
Diet
Avoid iron supplements and iron-fortified cereals; tea with meals reduces iron absorption; limit vitamin C with high-iron meals.
Supplements
Avoid iron and high-dose vitamin C; standard multivitamins should be iron-free in expressing carriers.
Peptides
Drugs
Therapeutic phlebotomy primary; iron chelators (deferasirox, deferoxamine) reserved for those unable to tolerate phlebotomy.
