Hemoglobin A1c (HbA1c)

Routine annual

also glycated hemoglobin, a1c, glycohemoglobin

What it isA1c is the percentage of hemoglobin molecules inside red blood cells that have sugar permanently bonded to them. Because red blood cells live about 120 days, A1c reflects the average ambient glucose load over roughly the previous three months.

DoesIt acts as a time-integrated record of blood sugar — not a snapshot. Every glucose excursion leaves a mark, and that mark only clears as old red cells get replaced.

GoodLow A1c indicates stable, low-amplitude glucose handling and predicts a long disease-free runway across cardiovascular, cognitive, and metabolic domains.

BadHigh A1c indicates chronic hyperglycemia and tracks tightly with diabetes, cardiovascular events, dementia, retinopathy, neuropathy, and accelerated aging. Even in the "normal" range, climbing A1c carries excess risk.

Standard
<5.7% normal; 5.7–6.4% prediabetes; ≥6.5% diabetes.
Optimal
4.6–4.9% (lower is good)
Modifiers
Anemia, hemoglobinopathies, and pregnancy distort A1c; high red cell turnover (athletes, altitude exposure) can falsely lower it; iron deficiency falsely raises it.

How to optimize

Lifestyle
Combine Zone 2 cardio with twice-weekly heavy resistance training to grow muscle (the body's largest glucose sink). Sleep 7–9 hours; even short-term sleep restriction raises A1c trajectories. Manage cortisol — chronic stress drives glucose up.
Diet
Mediterranean or low-carb whole-food template; cut sugar-sweetened beverages, ultra-processed grains, and seed oils. Eat protein and fiber before starch. Vinegar or olive oil pre-meal blunts spikes.
Supplements
Berberine (500mg 3×/day), magnesium glycinate (300–400mg/day), omega-3 EPA+DHA (2–3g/day), alpha-lipoic acid (600mg/day), inositol (myo + d-chiro, 2g/day).
Peptides
Tirzepatide, semaglutide, retatrutide produce dramatic A1c reductions (1–2.5% drops). MOTS-c improves insulin sensitivity.
Drugs
Metformin, GLP-1 agonists (semaglutide, tirzepatide, retatrutide), SGLT2 inhibitors, acarbose, pioglitazone.
